Equipment Problems, Checkout, and Management

If you find problems with any club equipment, please let us know by submitting a work order request.  If you want to CHECK OUT (borrow) or CHECK IN (return) a piece of equipment, complete the Equipment Check Out / Check In form.  In both cases, please provide adequate information for us to process these submissions as indicated on these form

We’ll receive the request and turn it into a work order.  If you are checking equipment out, you’ll receive an e-mail after we process your submission indicating that a work order has been assigned to you for checking out the equipment.  When you check equipment in, you should receive an e-mail closing the work order once it has been processed.
